This equipment will be placed in the room for you, it is not the responsibility of library staff to setup the equipment. If you need assistance and staff is available they will try to assist you.

Meetings must end fifteen minutes prior to the library’s closing time and all participants must vacate the building by closing time. Meetings are not permitted when the library is closed.

I hereby certify that this meeting conforms to all of the requirements specified in the attached Merriam Room Policy, and that I have correctly completed this application form to the best of my knowledge.

A signed copy of this agreement will be mailed to you providing proof that you have use of the room.
